Welcome to the Call of Duty 4 Team Death Match Tournament

This is the registration form for your team!

Please view the rules (click to download) before continuing *last updated: 20/03/2008 - 4:20pm

All the fields below are required this is to follow all proceedures for any up and coming tournaments held by 'The Xbox Lounge', 'cre4tion.com', and 'The Lair'

All matches & rules will be goverened by 'The Lair' their decision is final, please make sure you read the rules before you proceed, but submitting your team we assume you have read the rules and understand them. Once a ruling is made it is final!

Please make sure that you have your full team roster(min. 6 players) before applying for the tournament, Registrations will be open until 28th March 2008 (Friday). the tournament will then start on the 4th April 2008.

Please Note: Only captains can register their teams

Prizes will be handed out to each member of the winning team! the prize is a bundle and includes: COD4 Themed faceplate + COD4 Shirt
